SCHEDULING
Appointment times are available Monday through Friday. Our regular clinic hours are 9:00 AM to 5:00 PM Pacific Standard Time. Please make a special request if your schedule does not permit you to be seen within these hours and we will work with you to try and arrange a time that will best fit your needs. INSURANCE & PAYMENT OPTIONS
We accept most major insurance plans including Medicare and we can arrange convenient payment plans when necessary. Please call or use our Online Services to contact our Patient Services Administrator with any questions you may have regarding your insurance or financial needs.

WHAT TO EXPECT
As a new patient to our clinic you will be asked to complete a small series of new patient paperwork which will help with your financial and medical needs. Please arrive 10 to 15 minutes prior to your scheduled appointment time in order to complete them. Additionally, we request that you bring:

• Any pertinent medical records you may have
• Name and location of your prior physician
• Insurance card(s)
